Inconsistencies between different parts of the text and the appendices have been retained.

The transcribed forms in Appendix 2 are intended to give approximations of the printed forms only; moreover, depending on the hard- and software used, some of the forms may be too wide to display as intended. The forms are therefore also included as illustrations (a hyperlink has been provided where relevant).
